You are requested to annually update the sites where you serve as medical director by completing the form found online at https://paltc.org/facilityform.  You will be sent annual reminders to complete this form. Please take a moment now to enter the sites of service where you currently serve as medical director.  This data is vital to AMDA and ABPLM efforts to provide information on outcomes and the value of certification specialized training and verification in Post-Acute and Long-Term Care Medicine.

As of January 1, 2020, your CMD status will be a matter of public record unless you opt out in writing. A posting of all current CMDs with initial certification and recertification dates will be available on the www.abplm.org website as of January 1, 2020. If for any reason, you do not want your credential to be a matter of public record, send a written request to opt out of this posting to cmd@paltc.org by November 1, 2019. Written verification of your CMD status may be obtained by downloading the verification form online at https://www.abplm.org/certification-verification or by contacting the ABPLM at cmd@paltc.org
