Certification Verification for CMDs

The Certified Medical Director (CMD) credential is a voluntary certification. CMDs listed below have given permission to post their CMD status. Scroll through the table to find a physician by name. Where a recertification date appears in the past, the CMD is in a grace period to complete documentation of recertification eligibility.  To request written verification of a CMD or to verify the status of a Certified Medical Director's (CMD) credential whose name does not appear in the database below, please download the certification verification request form.  The form must be signed by the physician for whom verification is sought and returned to cmd@paltc.org or via fax to 888-249-6533.
