CMD Certification Applications
The American Board of Post-Acute and Long-Term Care Medicine (ABPLM) currently administers a certification program for Certified Medical Directors (CMD) in post-acute and long-term care settings. Certification is valid for six years.
Application Review Fees:
Initial Certification $525 (AMDA PALTC members), $625 (non-members)
Recertification $400 (AMDA PALTC members), $500 (non-members)

Download and save a copy of the application to your computer. Print out completed applications to add signatures then fax, or scan and e-mail the completed form to the ABPLM. Application fees may be paid via credit card using the form within the application, If you must pay by check, mail the check with the fee payment page to:
