CMD Certification Applications


The American Board of Post-Acute and Long-Term Care Medicine (ABPLM) currently administers a certification program for Certified Medical Directors (CMD) in post-acute and long-term care settings.   Certification is valid for six years. Click the links below for information on: eligibility pathways, initial and recertification eligibility requirements, and for applications for initial or recertification. 


Download and save a copy of the application to your computer.  Print out completed applications to add signatures then mail, fax, or scan and e-mail the completed form to the ABPLM at:

CMDs whose certification has lapsed may apply for reinstatement via a waiver request and sumbission of a reinstatment application. Please contact the ABPLM Director to discuss reinstatment eligibility process.
