CMD Certification Applications
The American Board of Post-Acute and Long-Term Care Medicine (ABPLM) currently administers a certification program for Certified Medical Directors (CMD) in post-acute and long-term care settings. Certification is valid for six years. Click the links below for information on: eligibility pathways, initial and recertification eligibility requirements, and for applications for initial or recertification.
Download and save a copy of the application to your computer. Print out completed applications to add signatures then fax, or scan and e-mail the completed form to the ABPLM. Due to the Covid-19 Crisis, you are asked not to mail physical applications as we are working remotely. Application fees may be paid via credit card using the form within the application, If you must pay by check, please send the check separately to the address below.

CMDs whose certification has lapsed may apply for reinstatement via a waiver request and sumbission of a reinstatment application. Please contact the ABPLM Director to discuss reinstatment eligibility process.
